Chemistry

Viagra

Sildenafil citrate is designated in chemical formula as citrate 1 – [[3- (6,7-dihydro-1-methyl-7-oxo-3-propyl-1H-pyrazolo [4,3-d] pyrimidine-5-yl] ) -4-ethoxyphenyl] sulfonyl] -4-methylpiperazine. Sildenafil citrate is a white or almost white crystalline powder with a solubility of 3.5 mg / ml in water and a molecular weight of 666.7 g / mol. Viagra (Sildenafil Citrate) is manufactured as a blue, diamond-shaped tablet with round corners, equivalent to 25 mg, 50 mg and 100 mg of sildenafil for oral administration. In addition to the active ingredient, each tablet contains the following inactive ingredients: microcrystalline cellulose, calcium dibasic anhydrous phosphate, croscarmellose sodium, magnesium stearate, titanium dioxide, lactose, triacetin, and Blue # 2 (Aluminum Lake) dye.

Levitra

The main ingredient of Levitra is Vardenafil, which is a piperazine, whose chemical formula is monohydrochloride 1 – [[3- (1,4-dihydro-5-methyl-4-oxo-7-propylimidazo [5,1-f]] 1,2,4] triazin-2-yl) -4-ethoxyphenyl] sulfonyl] -4-ethyl. This is an almost pale and solid compound with a molecular weight of 579.1 g / mol and a solubility of 0.11 mg / ml in water. Apart from the main ingredient. a tablet of Levitra contains crospovidone, microcrystalline cellulose, colloidal silicon dioxide, hypromellose, polyethylene glycol, magnesium stearate, yellow ferric oxide, titanium dioxide and red ferric oxide. Below is a diagram showing the structure of Vardenafil hydrochloride:

Prescribed for

Viagra: Sildenafil is used to treat erectile dysfunction of organic (medical condition) or psychogenic (psychological) origin and to treat pulmonary artery hypertension.

Levitra: Vardenafil is used to treat impotence.

Dose

Viagra: For most people, the recommended dose is 50 mg, taken as needed, approximately 1 hour before sexual activity. However, Viagra can be taken 30 minutes to 4 hours before sexual activity. Based on effectiveness and tolerance, the dose can be increased to the recommended maximum of 100 mg or reduced to 25 mg. The maximum recommended dose should not be taken more than once a day.

Levitra: For most people, the recommended initial dose of Levitra is 10 mg, taken orally as 1 hour before starting sexual activity. The dose can be increased to a maximum of 20 mg or reduced to 5 mg based on effectiveness and side effects. The maximum dose should not be taken more than once a day. Levitra can be taken with or without food. Sexual stimulation is required to respond to treatment.

When should it be taken?

Take the medicine 1 hour before sexual activity. These medications are taken as needed. No more than one dose should be taken every 24 hours, except under medical instructions.

Storage

It should be stored at room temperature, but not in the kitchen or bathroom. Avoid exposing these medications to moisture or direct sunlight.

Mechanism of action

Viagra: More than half of men with impotence can have a physical (medical) cause for their condition. The rest are believed to have psychological causes for their impotence. The medical causes of impotence include diabetes , and circulatory, neurological and urological conditions. Erection of the penis is caused by the entrance of blood to the penis. Enlargement occurs when the arteries that carry blood to the penis increase in flow and the veins that allow the blood to return to the body diminish their capacity. Under normal conditions, sexual stimulation causes the production and emission of nitric oxide in the penis. Nitric oxide then activates an enzyme, called guanylate cyclase, which causes the production of cyclic guanosine monophosphate ( cGMP), in English). It is the cGMP the main responsible of the erection to affect the amount of blood that the arteries and veins add and remove from the penis. Sildenafil inhibits an enzyme called phosphodiesterase-5 ( PDE5 ) that destroys cGMP. In this way, sildenafil prevents the cGMP from being destroyed and allows the cGMP to accumulate and take effect for a longer time, extending the ease and duration of the erection. Sildenafil also reduces the pressure inside the pulmonary artery, treating a serious condition called pulmonary artery hypertension.

Levitra: An erection occurs when blood enters the penis. This happens when the arteries are enlarged and the blood flow increases while the veins that control the exit of the blood from the penis are diminished. With sexual stimulation, nitric oxide is produced in the penis, which activates the enzyme called guanylate cyclase, and this in turn causes the production of cyclic guanosine monophosphate (cGMP). The CGMP directly governs the enlargement and reduction of blood vessels in the penis. Vardenafil prevents the enzyme called phosphodiesterase-5 (PDE5) from destroying cGMP, so that its effect persists for longer. The longer the cGMP persists, the longer the erection will be.

Side effects

Viagra: As with all medications, people who take Viagra may experience side effects. These are usually mild and do not last more than a few hours. The most common side effects are:

  • Headache
  • Redness of the face
  • Stomach ache

Less common side effects include blurred vision or blue tones, or sensitivity to light. In clinical studies:

  • Patients rarely stop taking Viagra because of the side effects.
  • The frequency of side effects shows that it decreases with time.

Taking Viagra does not make the person more dependent to have or maintain an erection.

Levitra: The most common side effects of Levitra are:

  • Sudden heat Using several removable pieces of clothing, summer clothes or being in a warm environment can help reduce this effect.
  • Mild dizziness
  • Headache
  • Nasal congestion
  • Stomach ache
  • Sensitivity to light
  • Bluish tones in vision or other visual abnormalities

In very rare cases, people who have used PDE5 inhibitors, such as Levitra, have experienced a sudden loss of vision that may be temporary or permanent. The cause of this effect has not yet been determined. It has not been confirmed if the medication is responsible for the loss of vision.

Do not take these medications if …

  • You have an allergy to vardenafil or another ingredient of the medication
  • The doctor should be told if the person is allergic to other medications. The reactions that one has in the allergy must be specified. This includes skin rashes, itching, shortness of breath, cough, swelling of the face, tongue or throat and any other symptoms presented.
  • If you are taking any of these medications: isosorbide dinitrate, isosorbide mononitrate or nitroglycerin.
  • If the person is using stimulants such as nitrate amyl or nitrate butyl.
  • If the doctor has told the patient to avoid sexual activity.

Precautions

  • If the person suffers from heart problems, you should consult with your doctor before taking these medications.
  • Check the use of other medications with a doctor. Viagra and Levitra may have negative interactions with other medications.
  • These medications should not be taken with other products used to change sexual desire or ability.
  • You should avoid consuming grapefruit or juice / grapefruit juice.
  • Avoid alcoholic beverages, including beer, wine and liquor.
  • To protect against sexually transmitted diseases, the use of condoms (prophylactics) is recommended.

Drug interactions

Viagra: Sildenafil increases the effect of medications that lower blood pressure. It also increases the blood pressure reducing effects of nitrates, for example, isosorbide dinitrate (Isordil), isosorbide mononitrate (Imdur, Ismo, Monoket) and nitroglycerin (Nitro-Dur, Transderm-Nitro), mostly used to treat angina. Patients taking nitrates should not take sildenafil. Cimetidine (Tagamet), erythromycin, ketoconazole (Nizoral), itraconazole (Sporanox) and mibefradil (Posicor) can cause a marked increase in the level of sildenafil in the body. Patients who are taking these medications should be observed carefully if they take sildenafil. It is estimated that rifampin decreases the levels of sildenafil in the blood and probably reduces its effectiveness.

Levitra:The processing and elimination of vardnafil from the body is inhibited with erythromycin, ketoconazole (Nizoral), itraconazole (Sporanox), indinavir (Crixivan) and ritonavir (Norvir). Therefore, these medications increase the concentration of vardenafil in the blood. Therefore it is recommended that if these drugs are combined, the dose of vardenafil is reduced to avoid side effects. Vardenafil reduces the concentration of ritonavir and indinavir and may reduce the effects of these medications. Vardenafil increases heart rate and exaggerates the blood pressure lowering effects of nitrates (such as nitroglycerin). In patients taking nitrates for chest pain (angina), vardenafil can cause chest pain by increasing the heart rate and lowering blood pressure. Thus, Patients with angina should not use vardenafil. Vardenafil also exaggerates the blood pressure lowering effects of medications called “alpha blockers,” such as terazosin (Hytrin) and should not be used by people who take alpha blockers.
